A new menace has emerged in town—her name is Nina. Once just a devoted fan of Jeff the Killer, her sanity unraveled following a tragic incident during her school years, when relentless bullying targeted her and her brother. In a desperate act of revenge, she murdered the bullies and transformed her appearance by bleaching her face, turning her skin ghostly pale. The game features two distinct story modes: assume the role of PoisonGuy and hunt Nina down, or step into the shoes of Nina the Killer herself, striving to escape from the asylum.
